It seems unusual that the match it greater than the amount of funding to be awarded. We'd wondered why the match isn't, at least, the same amount of the grant award. The initial provision of services was from a sole source however due to staff changes, they ae not able to provide the needed services. It was also their commitment to provide $155,957 of in kind. Our grant award is based on this amount of in kind. Is there an anticipated funding source or continuation plan for the program after the grant period ends if outcomes are successful? This program is designed with long-term sustainability in mind. In addition to initial funding, CSSWFNY plans to: • Seek additional partnerships with local employers, government agencies, and community organizations to continue funding and support. • Pursue federal, state, and private funding opportunities as they become available to maintain program operations and expand service offerings. • Use Workforce Innovation and Opportunity Act (WIOA) dollars, as appropriate, to absorb some of the future program costs What organizations will be responsible for referring participants to the Success Coach, and what is the estimated referral volume per year? Community organizations partnering on this initiative are: Steuben County Alcohol and Substance Abuse, Bridge to Hope Center, IHS, Casa Trinity, CCC, Alliance for Empowerment, AIM Independent Living Center, Catholic Charities of Steuben/Livingston, Arbor House and Capabilities. Referrals will come from these partners in addition to CSS's system programs. Referrals will go to CSS's Employment Specialist, who will vet referrals and then refer to the Success Coach. As the grant evolves it is expected that this community partnership will expand. Expected referrals to the Success Coach will be slightly over 60 per year if an agency is applying to cover Chemung, Schuyler and Steuben counties. If applying to cover just Schuyler and Chemung Counties, then referrals will be slightly over 30 per year. What is the expected caseload for the Success Coach and how many participants should be served annually? Expected caseload for the Success Coach will be slightly over 60 per year if an agency is applying to cover Chemung, Schuyler and Steuben counties. If applying to cover just Schuyler and Chemung Counties, then referrals will be slightly over 30 per year. What specific performance metrics will be used to evaluate program success (employment placement, retention, length of employment, etc.)? Businesses Served (Output) Businesses Improved (Outcome)

The Success Coach will not be expected to provide recovery-specific services but is to primarily connect participants to existing recovery and other needed resources in the community in order to stabilize their employment. The Success Coach is the liaison between the business and the employee, so establishing a good working partnership with the business is critical. The Success Coach will also assist the Employment Specialist in developing and implementing business workshops.
